POSITION SUMMARY
The Refugee Health Education and Outreach Liaison (Refugee Health Liaison) is responsible for providing health and mental health support and assistance with the goal of eliminating health related barriers to self-sufficiency and successful community integration. The Refugee Health Liaison connects clients to appropriate care and provides clients with educational resources to assist with healthcare system navigation to ensure that medical screening are conducted in a timely manner and that all clients establish primary care within the required time frame.

The Refugee Health Liaison facilitates health education and training for medical screening nurse staffs, for community services health staffs, for refugee resettlement case management staffs, and for refugees. He/She also ensures that the interpreter services for health encounters are done by trained interpreters.

Essential Functions Statement(s)

  • Maintain current knowledge of the U.S. Resettlement Program
  • Maintain current knowledge of all Refugee and Immigrant Services programs
  • Facilitate the timely conducting of the initial refugee medical screening through coordination with the Reception and Placement Program staff and the local health district nurse staff
  • Facilitate the provision of needed follow-up services through coordinating with the R&P staff and LHD staff
  • Facilitate the initial health screening to a primary care physician through coordination with the R&P staff, the LHD staff, and the Department of Medical Assistance Services (DMAS)
  • Facilitate the delivery of mental health services through the coordination between the resettlement office staff and the local Community Services Boards
  • Assist the resettlement staff with emergency refugee health needs
  • Facilitate training for (i) resettlement staff on health orientation for refugee; (ii) Community Services Boards
  • Provide guidance to clients on state and federal health insurance programs
  • Provides orientation to health care system
  • Interviews clients and assesses their health needs
  • Attend weekly Newcomer clinic at Health Department and or other contacted providers for coordination of services and follow-up
  • Ensures urgent health needs are met as soon as possible providing transportation as needed
  • Provides information and referral services
  • Accessible as a resource for information to Health Department staff during their home visits with newcomers
  • Identifies and refers problems for professional intervention
  • Provides support counseling and crisis intervention where necessary
  • Coordinates service provision with other staff, volunteers, health department, interpreters and medical offices
  • Problem resolution involving transportation, interpreters, emergency medical issues, rescheduling of appointments and communication of medical information.
  • Arranges or provides transportation for refugees to attend appointments at Health Department and health clinics
  • Assists in providing education on the use of Medicaid taxi 
  • Maintain accurate records of all services provided
  • Produce reports in a timely manner
  • Work as part of the program team and participate in staff meetings and in-service training
  • Conduct periodic review of program case files
  • Outreach and Education: conduct health orientations to newly arrived refugees as part of local cultural orientation program and provide written information on health topics as needed
  • Develop relationships with health care providers in the community and coordinates with community agencies and associations that provide health care non-English speakers.
  • Access health education for refugees and immigrants with chronic health conditions clients that are hospitalized and meet with discharge planner before they are discharged in home visits to clients upon release from the hospital
  • Advocate for clients needs
  • Coordinate space, transportation, and child care plan incentives for attendance to group meetings
  • Develop dental resources for care of children and adults
  • Documents the following in electronic case notes: contact and health services provided to clients based upon the needs and goals identified in the resettlement plan monitoring progress of members of the case over time regard to health issues
  • Maintains detailed flies of service providers for health care, health education, transportation and interpretation
  • Prepare and submit reports as directed

SKILLS & ABILITIES


Education:
Bachelor's Degree (four year college or technical school) Required, Field of Study: Health or Human Services or related field

Experience:
2 plus years of experience in human services; public health

Computer Skills:

  • Intermediate hardware skills - navigating, saving work, searching Internet
  • Intermediate software skills - Microsoft Applications (Access, Excel, Word, Power Point)

Other Requirements:
