OVERVIEW STATEMENT OF POSITION DUTIES
Under general supervision, aids social workers by locating resources needed for families, transporting families to appointments.
DUTIES AND RESPONSIBILITIES

- Duty #1 50% Accompanies clients to medical appointments and on the client's behalf, may consult with medical personnel. Assists clients with errands such as shopping, including providing guidance on their purchases. May accompany clients to senior centers, Adult Day Care Centers, and other appointments. May deliver sensitive client information to other service providers.
- Duty #2 25% Shares observations and information obtained about the clients with the assigned social worker. Collects and maintains program statistics. Assists with the preparation or statistical and narrative reports. Assists social workers with locating resources for clients such as food, furniture, clothing, rental and utility assistance, and moving storage. Maintains documentation about availability of resources and those that are obtained for clients. Assists social workers with client check-in calls and provide documentation, follow up and referral as needed.
- Duty #3 25% Performs other duties as assigned in support of the Adult and Aging division and the Department of Family Services including but not limited to assisting with administrative tasks, documentation in electronic records, filing of required documents in electronic and paper charts, and driving agency vehicles to County facilities for repair and maintenance.
